CLINICAL FOLDER REQUIREMENTS TIMELINE
Due by the end of MAST 103:
• Updated MSU ID (this must be updated each semester)

• Lab coat (white)

• Scrubs and white shoes

• Clinical Pack (purchased during MAST 103 course and used throughout
MAST courses).

• Color photo of student (passport or wallet size). *This is also a Program
requirement*

• Proof of Health Insurance

• Notarized copy of valid driver’s license

• Completed Health Questionnaire (Instructor will provide form)

• Proof of AAMA membership (Instructor will provide form)


I have my white shoes already. I guess its a start!! lol Yeah I really need to get busy!
